Assignment Description SELECTION FOR CONSULTANTS BY THE WORLD BANK GROUP REQUEST FOR EXPRESSIONS OF INTEREST Electronic Submissions through World Bank Group eConsultant2 https://wbgeconsult2.worldbank.org/wbgec/index.html ASSIGNMENT OVERVIEW Assignment Title: 1274403 - Rwanda Stunting Prevention and Reduction Program - Facility survey Assignment Countries: - Rwanda ASSIGNMENT DESCRIPTION The objective of the Rwanda Stunting Prevention and Reduction Program is to support Rwanda's vision and commitment to eliminate chronic malnutrition in children under two through the scale-up of a combination of nutrition-specific and nutrition-sensitive interventions, addressing both the supply side (nutrition services) and the demand side (CCTs). The facility survey involves 4 activities: (1) a facility questionnaire to collect data on the availability and readiness for nutrition services; (2) health worker interviews; (3) exit interviews with attending ANC and growth promotion services; and (4) extraction of historical birthweight information from delivery registers. FUNDING SOURCE The World Bank Group intends to finance the assignment / services described below with trust fund resources provided by the Bill and Melinda Gates Foundation (BMGF), Power of Nutrition (PON), and Child Investment Fund Foundation (CIFF). ELIGIBILITY Eligibility restrictions apply: 1. Experience in the core business of data-collection and surveys. 2. Qualifications and Relevant technical and managerial capabilities of key staff. 3. Terms of Reference Rwanda Stunting Prevention and Reduction Program Impact Evaluation Baseline Facility Survey Firm TOR April 2021 PURPOSE OF THIS CONSULTANCY The purpose of this consultancy is to manage all aspects of the baseline facility survey - data collection, entry, management, and survey report - for a prospective impact evaluation of the Rwanda Stunting Prevention and Reduction Program (SPaR). Details on the survey activities and related scope of work are delineated in the relevant sections below. BACKGROUND ON THE STUNTING PREVENTION AND REDUCTION PROGRAM The Government of Rwanda, with support from The World Bank, has undertaken a multisectoral program with the aim of achieving significant reductions in stunting prevalence among children under 5 years of age, with a special focus on children under 2. The Rwanda Stunting Prevention and Reduction Program (SPRP) is comprised primarily of two projects: a set of health sector focused, supply side interventions in 13 districts under the Stunting Prevention and Reduction Project (SPRP); and the Nutrition-Sensitive Direct Support (NSDS) in 18 districts (5 additional to the 13 under SPRP), a demand-side cash transfer program with co-responsibilities for health service utilization under the Strengthening Social Protection Project (SSPP) . The theory of change hypothesizes that an integrated multisectoral program focused on a complementary set of demand side and supply side interventions primarily in health and social protection in the highest stunting districts and most vulnerable households will increase the quality, coverage, and utilization of health and nutrition interventions, enhance access to food and nutrition services, improve child caring, feeding, and hygiene practices, and lead to improved child development outcomes and reductions in stunting. The core interventions under the SPRP project are performance-based financing schemes at health centers and for community health workers, a national behavior change campaign, complementary support for WASH in the poorest households, support for a home-based early childhood development program, as well as the distribution of fortified blended foods (FBF) for mothers and children living in the most vulnerable households Under the SSPP, the cash transfer program will target households in the lowest socio-economic quartiles with children under 24 months of age and/or pregnant and lactating women. In addition to the SSPP and SPRP projects, nutrition-sensitive interventions will also be supported through the Sustainable Agricultural Intensification for Improved Livelihoods, Food Security and Nutrition Project (SAIP) which includes economic empowerment of women and enhancing availability and consumption of nutritious foods. Innovations under the agriculture projects will be carried out in 3 of the 13 districts targeted for support under the SPRP and SSPP and will cover one sector and a small proportion of the villages in each district. Given the limited scope of the agriculture project, the impact evaluation will focus on the effects of the SPRP and SSPP and will exclude intervention sectors in the 3 SAIP districts. THE IMPACT EVALUATION The prospective impact evaluation of the SPaR program addresses a series of (nested) research questions concerning the effectiveness of either the overall program or of components of the program. Specifically, the evaluation plans to investigate the following three questions: 1. What is the net impact of the total program effort on the priority outcomes of interest among children under five years of age, with a special focus on children under two years of age for the most vulnerable households (Ubudehe 1)? 2. What is the impact of the nutrition focused CCT on child stunting levels in targeted households (Ubudehe 1)? 3. Is there a differential impact of the CCT on targeted households in the districts not accompanied by the SPaR health system strengthening activities? The primary outcome for all interventions in the impact evaluation are height-for-age in children under 2 years of age. Secondary outcomes will be selected to trace mechanisms through which the intervention operates, such as utilization of key services/interventions for young children and pregnant women and will include child development scores and community health worker knowledge. An advisory impact evaluation team composed of evaluation experts with backgrounds in economics, nutrition, and public health, has been constituted to advise the GoR on the design, conduct, and analysis of the impact evaluation. During the implementation of the baseline facility survey, the advisory team will be supported by an in-country consultant whose role would be to provide ongoing support to the survey team to ensure the quality of the data collection process and the data, support capacity building, and provide timely feedback for corrective action when necessary. THE FACILITY SURVEY DESIGN AND STUDY SAMPLE COMPOSITION Strengthening the delivery of nutrition services at health facilities is a significant component of the SPaR program interventions and a key pathway in the results chain for ultimately improving nutrition outcomes of children under 5. There are three primary aims for the facility survey: 1. To assess the provision and quality of antenatal and growth monitoring and promotion services, with a focus on nutrition, and how these vary according to the SPaR intervention component received, and between data collection rounds. The assessment of service quality will include components on provider knowledge, content of care, patient satisfaction, as well as provider trainings, supervisions, and structural quality. 2.
